Q- A young man complains of syncope, dyspnea and chest pain after exercise. A crescendo-decrescendo systolic murmur is heard over the left sternal border without radiation. What is the most likely etiology?
A- Aortic dissection
B- Aortic regurgitation
C- Aortic stenosis
D- Hypertrophic cardiomyopathy
D- Hypertrophic cardiomyopathy- Hypertrophic cardiomyopathy typically presents in young patients with syncope, dyspnea, and chest pain during exercise. The murmur is a crescendo–decrescendo systolic murmur best heard at the left sternal border, without carotid radiation, as seen in aortic stenosis.
Q- A 9-month-old who was delivered after premature labor and is being fed on cow milk and orange juice. What is the predicted deficiency?
A- Calcium
B- Iron
C- Vitamin C
D-Vitamin D
B- Iron- Cow’s milk is low in iron and can also cause microscopic GI blood loss in infants, leading to iron deficiency. Premature infants are already at higher risk because they miss the period of rapid third-trimester iron accumulation. Orange juice does not correct this deficit.
Q- Which of the following is the best next step for a suspicious malignant melanoma?
A- Excisional biopsy
B- Excision with a safety margin
C- Observation
D- Shave biopsy
A- Excisional biopsy- For a lesion suspicious for malignant melanoma, the best next step is excisional biopsy with narrow margins to allow accurate diagnosis and depth measurement (Breslow thickness). Wide excision is done only after confirming the diagnosis. Shave biopsy and observation are inappropriate for suspected melanoma.
Hello Sir.
Would it be possible to send me all USMLE questions you are posting?, i would be thankful
Dr. Abid, excisional biopsy when all the tumor is removed and sent to the lab as in malignant melanoma and most of skin cancers. Incisional when a part of it is removed and sent. Wedge biopsy is considered when the lesion is removed with a wedge of normal surrounding tissue. I wish this answers your question. Thank you Dr.
Sir please give me list of important cases where excisional , incisional and wedge biopsy is performed. These terms confuse me alot in questions
Thank you. I really appreciate your encouraging feedback.
I found these questions very supportive and i want to thank you for your role in devlopong physicians skill and knowledge across the glob especially for us in deprived and poor countries .
