D- Progesterone receptors- Mifepristone is an antagonist at progesterone and glucocorticoid receptors. It stimulates uterine contractility and increases prostaglandins through inhibition of prostaglandin dehydrogenase. It also blocks cortisol receptor binding and reduces excess cortisol effects is cases of Cushing syndrome.



Q- A patient is diagnosed with high blood pressure and prescribed hydrochlorothiazide. Which one of the following is a common side effect of this drug?

A- Constipation

B- Low blood glucose

C- Potassium wasting

D- Seizures

C- Potassium wasting- Thiazides diuretics act by lowering the reabsorption of sodium at the distal convoluted tubules. This leads to increased production of aldosterone which releases potassium through a compensatory effect. This mechanism results in potassium wasting. Other side effects of hydrochlorothiazide are high levels of blood glucose, uric acid and lipids.



Q- A patient is prescribed nitroglycerin sublingually for angina. Which one of the following drugs may interact to result in severe hypotension if taken with nitroglycerin?

C- Sildenafil- When both drugs are administered at the same time, accumulation of excessive amounts of cGMP occurs which leads to severe hypotension. Other drugs have no significant interaction with nitroglycerin.
